This document should outline your circumstances, the period affected, and how your studies were impacted. Providing clear, professional evidence ensures your request can be assessed fairly and in line with Ikon’s Assessment and Reasonable Adjustment Policies.
What makes a medical certificate or letter acceptable
Your certificate or letter must:
- Be issued by a qualified professional (e.g., registered doctor, counsellor, psychologist, or lawyer).
- Be written on official letterhead, including the provider’s name, registration or licence number, and contact details.
- Specify the dates of consultation and the duration or impact period on your studies.
- Clearly explain how your condition or circumstance affected your ability to complete your assessments or participate in study.
- Be signed and dated by the professional.
- Be written in English, or include a certified translation if originally in another language.
Note: If you prefer not to disclose specific medical details, your practitioner may provide a statement outlining how your condition affected your studies instead of describing the diagnosis.
Translation requirements
If your certificate or letter is not in English, it must be translated by a NAATI-accredited translator. Accredited translators can be found at NAATI.
Evidence standards
To be accepted, your document must:
- Be verifiable — Ikon may contact the issuing provider to confirm authenticity.
- Include your full name, dates of impact, and a clear description of the effect on your study performance.
- Be submitted in PDF or JPG format (copies only — keep your originals safe).
All evidence is treated confidentially and managed in accordance with Ikon’s Privacy and Assessment Policies.
